Small Baller Policies
Our policies are considered read and accepted upon enrollment by clicking the provided button and statement. Please read through them and if you have any questions contact us for clarity. We are always here to help.
Every policy is in effect at the start of the first class of each 8 week session.
There are 11 small policies below covering the areas of use, participation, payment, use of site, use of facilities and also health and safety including outbreak protocols, in paritcular with Covid 19.
It is being point out here as well that only one parent per child is to attend the facility with the Small Baller participant. We cannot have additional family members and other children not participants on location as it is very distracting and consumes precious time of the class. We trust you fully understand and that this policy cannot be ignored.
Administration Fee $139 Per Session All Class payments are due in advance of the start of the season. Drop classes are NOT PERMITTED.
No Refund Policy: All payments are final and are non refundable. If you decide to discontinue after having attended a class the No Refund Policy is in effect.Â
Payment Options: You can pay at the time classes start using our wireless unit or you can pay at the time of registration. If paying in person you may also use cash. All fees must be paid in advance of the start of the first class. Spots will not be guaranteed if not.
The undersigned does hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Small Ballers and its member institutions, or any subdivision thereof, and each of them, their officers and employees, (collectively, the “Releases'”), from and for any liability resulting from any personal injury, accident or illness (including death), and/or property loss, however caused, arising from, or in any way related to, Participant’s participation in Small Baller Class, except for those caused by the willful misconduct, gross negligence or intentional torts of the above parties, as applicable.
Indemnification and Hold Harmless: The undersigned also hereby agree to INDEMNIFY, DEFEND AND HOLD the Releases’ HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities including, but not limited to, attorney’s fees, arising from, or in any way related to, Participant’s participation in Small Ballers’ Class, except for those arising out of the willful misconduct, gross negligence or intentional torts of the above parties, as applicable.
In the event of a medical emergency we will immediately contact the named parent, if they are not on site, then thereafter notify the appropriate emergency services to respond. In the event of any health issue, we will respond according to the provided medical history on file (as provided at the time of registration) and notify appropriate emergency responders.
The undersigned further agree to allow, without compensation, Participant’s likeness and/or first name only, to appear, and to otherwise be used, in material, regardless of media form, promoting the Small Ballers, and/or its camps, Classes or any activities, including those of its representatives and licensees.
Small Ballers reserves the right to change these policies at any time in accordance with facility requirements or administrative changes as they may be necessary from time to time. This does not change any acceptance in advance of said changes and all Parents are reminded to review our Policies.
The undersigned have read this assumption of risk, release and waiver of liability and indemnity agreement, and have had the opportunity to ask questions about the same. The undersigned fully understand this assumption of risk, release and waiver of liability and indemnity agreement, that the undersigned are giving up substantial rights in connection therewith, and that its terms are contractual, and not a mere recital. The undersigned acknowledge that they are signing this agreement freely and voluntarily
The undersigned expressly agree that the foregoing assumption of risk, release and waiver of liability and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province of Ontario and the town of Bradford, or any town wherein the Small Baller program is operating within the province of Ontario, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I give Small Ballers and their employees permission to check mine and my child’s temperature. This agreement will remain in force as long as and whenever I/we participate in any activity with Small Ballers.
I give Small Ballers and their employees permission to check my child, the named registrants’, temperature. I further give my child, permission to use hand sanitizer while at Small Ballers. This agreement will remain in force as long as and whenever my child participates in any activity with Small Ballers on premises or away from premises while with the program.
The undersigned hereby acknowledge and agree that they understand the nature of Small Ballers Club; that the Participant is qualified, in good health, and in proper physical condition to participate therein; that there are certain inherent risks and dangers associated with Small Ballers Classes; and that, except as expressly set forth herein, they, knowingly and voluntarily, accept, and assume responsibility for, each of these risks and dangers, and all other risks and dangers that could arise out of, or occur during, Participant’s participation in the Small Ballers Class.
